What is in a Transition Plan?

At the age of 16 (or younger if recommended by the IEP team), IDEA 2004 requires that students and their IEP teams select a "course of study" in order to begin the Transition process.   (NOTE:  Pennsylvania regulations require a transition planning begins at age 14).  Additionally, the student’s IEP is required by law to have specific components related to Transition. 

·        Measurable postsecondary goals

·        Student interests, preferences, strengths, and needs

·        Transition activities including:

o       Instruction

o       Related services

o       Community experiences

o       Development of employment skills

o       Other adult objectives and,

o       A listing of the person/agency responsible for such activity.
